Rebensburg was in the leader's box when Brem, who had been quickest after the first run, began her second descent through the gates needing to finish no lower than fourth to secure the giant slalom globe.
She managed it but it was close as Brem finished 0.04 seconds quicker than Marie-Michele Gagnon to push the Canadian down to fifth on the day and finish two points better off than Rebensburg in the final standings.
"It was an amazing fight this season in the giant slalom and to have it come down to this final race and such a small margin is unbelievable," said Brem.
"I couldn't be happier with how everything worked out today."
Rebensburg took the race, the finale to the World Cup season, by 0.46 seconds from France's Taina Barioz after a storming second run that was a full second quicker than anyone else pushed her up from seventh after the first leg.
Switzerland's overall World Cup winner Lara Gut finished in third place. Gut topped the final overall standings by nearly 300 points from injured American Lindsey Vonn, with Rebensburg in third place.
Sweden's Andre Myhrer concluded the men's season with victory in the final slalom race. Slalom globe winner Henrik Kristoffersen of Norway could manage no higher than 19th.
The Swiss resort of St Moritz will also host next year's world championships.
