Fanchini's last triumph came nine years and 45 days ago when she took gold in the downhill held at Lake Louise in Canada.
But she was finally enjoying the winning feeling again as she clocked 1:09.53 from her run to take victory in front of her home fans.
Second went to Canadian Larisa Yurkiw who finished 0.15 seconds back, while Germany's Viktoria Rebensburg completed the podium in 1:09.84.
Lindsey Vonn, Olympic downhill champion in 2010, was unable to trouble the leaders in the foggy conditions, placing tenth although she retains the World Cup downhill leaders bib.
While it was also a disappointing day for overall World Cup leader Tina Maze who finished further down in 13th.
