Adam Nelson of the United States was second with 21.26 to add to the silver he won two years ago in Edmonton, while Yuriy Bilonog of Ukraine was third with 21.10.
Defending champion John Godina could only manage 19.84 metres for ninth and quit the competition after his third attempt was a foul -- his second of the final.
"It was one of those days when things don't go your way," said the 31-year-old American, who had produced the longest distance in qualifying with 21.08.
"I put myself in a bad position with two bad throws, but I didn't think my third (attempt) was a foul."
Mikhnevich, who was disqualified from the 2001 world championships after failing a dope test, had five puts over 21 metres. His other attempt was a foul.
U.S champion Kevin Toth, whose personal best of 22.67 metres, set in April, was the top performance in the world since 1990, finished fourth with 20.89.
