Trott departed the 2013/14 Ashes with a condition that was later diagnosed as situational anxiety but has rebuilt his career at Warwickshire and earned a 50th cap in the first Test against West Indies at the Sir Vivian Richards Stadium in Antigua.
Cook lost the toss but was surprisingly asked to bat first by home skipper Denesh Ramdin, taking guard alongside Trott.
James Anderson was also included for his 100th Test appearance and needs four wickets in the match to usurp Sir Ian Botham's record of 383 wickets for England.
Barbados-born Chris Jordan was named as England's third seamer, edging out Liam Plunkett and Mark Wood.
