Team Sky's Richie Porte had been expected to make up ground on overall leader Alberto Contador during the 200km flat stage from Civitanova Marche to Forli but a mechanical problem ruined the Australian's day and he trailed in one minute five seconds after the winner.
Boem, who had been in the breakaway, pipped fellow Italian Matteo Busato to the finish line to win the stage in four hours 26 minutes and 16 seconds.
Former Tour de France champion Contador maintained his three second lead over Astana rider Fabio Aru in the overall standings while Porte, who had been third at the start of the day, dropped down to fourth after losing around 45 seconds. Spaniard Mikel Landa, also from Astana, climbed to third.
"It's never an easy day, seeing Richie Porte's case, nothing ever goes to plan. I've had my bad moment and Richie's had his bad moment today," Tinkoff - Saxo rider Contador, who dislocated his shoulder following a high speed pile up during last week's sixth stage, told Eurosport.
