The 16-year-old, making her worlds debut after previously securing the European title, was flawless in her routine and was rewarded with 150.10 points from the judges, giving her an overall total of 223.86.
Medvedeva said: “I don’t have any emotions right now, I left everything on the ice.
“I don’t think I will realize quickly that I won today because one year ago I was still skating in juniors.”
Ashley Wagner ended America’s 10-year wait for a medal with silver, scoring a personal best 142.23 points in the free-skate for a combined 215.39 with Russia’s Anna Pogorilaya (213.69) completing the podium.
Wagner, the final skater to take to the ice, said: This is absolutely incredible because that was the hardest thing I’ve ever had to do.
“To go out there feeling the way I did, being as terrified as I was and finding a way to get into the ice and have access to the skater I’ve been in training, that is unbelievable.”
In the pairs competition, Meagan Duhamel and Eric Radford of Canada usurped Chinese pair Winjing Sui and Cong Han to win gold.
Second following the short program, the Canadian duo posted a personal best 153.81 for a 231.99 total to ensure they successfully defended their title.
An impressive routine included a throw quadruple Salchow, throw triple flip and four level-four elements.
Duhamel said: “It was an out of body experience.
“We knew we were better than what we’ve done this season. We wanted to come here and do it for ourselves.
“Back in October at the Grand Prix, I thought we were out of contention for a medal at Worlds, I thought we would be fighting for fourth or fifth, so what a turnaround we made.
“It feels so unbelievable.”
Germany’s Aliona Savchenko and Bruno Massot finished third, beating Russia’s Ksenia Stolbova and Fedor Klimov to the bronze.
