"Chelsea Football Club has today agreed terms with Atletico Madrid for the transfer of Diego Costa," read a statement on the club's website.
"The transfer will be subject to the agreement of personal terms and a medical."
The Chelsea striker has not featured for the Blues this season following a summer in which he went on strike after being informed he was surplus to requirements by manager Antonio Conte.
"I'm not upset with anybody," Costa told Splash News in Sao Paulo.
"I have no bitterness towards anyone. Everything is cool.
"Everybody knows how much I love the fans."

Costa is known to be keen on a return to Atletico – the club he left to join Chelsea in 2014 for £32 million – and multiple reports on Thursday suggest that the deal is worth £57.2m fee for the player.
Costa is expected to have a medical in the coming days, with Atleti expecting to pay €55 million (£48 million) up front with €10 million (£9 million) in performance-related add ons.
Atletico are in the midst of a transfer ban, so Costa would be unable to feature for the club until January.

Costa played a pivotal role as Chelsea stormed to the 2016-17 Premier League title, finishing as the club's top scorer with 20 league goals.
But Conte reportedly informed Costa that he was no longer in his plans via text message over the summer, and the player has refused to return to training since, opting to stay in Brazil.
A move to Atletico would be a welcome return to La Liga for the 28-year-old as he looks to force his way back into the Spain national team ahead of next summer's World Cup.
