Milan are believed to have activated the €30 million release clause in Bacca's contract after missing out on his compatriot Jackson Martinez, who has moved from Porto to Atletico Madrid.
Bacca has spent two years with Sevilla, helping them win the Europa League twice and finish fifth in La Liga last season.
The former Club Brugge striker, who scored 49 goals in his two seasons with Sevilla, still has to complete a medical before the transfer is officially complete.
It's thought Bacca will sign a four-year deal and will earn around £50,000-per-week during his spell in Milan.
