The Japan No 1 seem to be cruising as he took the first two sets 6-3 6-4 before his Spanish opponent staged a remarkable comeback.
The 32-year-old won the third and fourth sets 6-3 6-2 to set up a thrilling finale.
However, he could not maintain the momentum as Nishikori, the world No 6, took the final set 6-2.
He will now face Richard Gasquet, who beat Nick Kyrgios earlier on Friday, in the next round.
