The Guatemalans, coached by Honduran Ramon 'The Primitive' Maradiaga, have four points from two games, one ahead of Mexico and the United States, who meet at the Azteca stadium on Sunday.
Costa Rica have three points, Panama one and Trinidad have yet to get off the mark.
Captain Guillermo Ramirez fired the home team in front with a right-foot shot in the 16th minute at the Mateo Flores stadium in Guatemala City.
Ruiz added two goals in the 29th and 37th minutes, taking his tally to nine in the qualifiers. Midfielder Carlos Edwards pulled one back in between for Trinidad.
Pezzarossi, known as 'The Tank', completed the Guatemala rout with two goals in a four-minute spell late in the second half.
Costa Rica overcame Panama in an eerily silent Saprissa stadium in San Jose, despite having midfielder Jose Luis Lopez sent off in the 47th minute for violent conduct.
The 'Ticos', ordered to play behind closed doors because of a missile-throwing incident in last month's 2-1 defeat by Mexico, went ahead when Wayne Wilson converted a penalty five minutes before halftime.
Panama, also attempting to reach their first World Cup finals, equalised from another penalty in the 58th minute, converted by Roberto Brown.
The visitors were poised to take a precious point back home until substitute Roy Myrie eluded several defenders to grab the winner in the 90th minute.
