Extra-time goals from Thomas Broich and Adil Chihi put the game beyond the reach of Schalke, who earlier had Zlatan Bajramovic sent off.
Cologne weathered the early storm from Mirko Slomka's side, who went close through Hamit Altintop and Marcelo Bordon early on, and took the lead in the 35th minute through an own goal from Dario Rodriguez.
The hosts doubled their advantage just a minute later, with Milivoje Novakovic applying the finishing touch to a well worked counter.
Matthias Scherz passed up two opportunities to put the game to bed just after half time, and Cologne were punished when substitute Peter Lovenkrands pulled a goal back after 55 minutes.
Bajramovic then saw red for retaliating, though Schalke still managed to haul themselves level, with Rodriguez atoning for his earlier own-goal with a header in the 75th minute.
The Uruguayan then missed his side's final chance of the game two minutes into extra-time, before Broich and Chihi sent the 50 000 crowd home happy, and Hanspeter Latour's side into the third round of the cup.
We're extremely disappointed," said Schalke coach Mirko Slomka. "We gave it everything but we played so badly in the first half that we deserved to lose."
HANNOVER PLUNGE DORTMUND INTO CRISIS
Hannover beat Borussia Dortmund 1-0 at Signal Iduna Park to leave Dortmund coach Bert van Marwijk hanging to his job by a thread.
Goalkeeper Roman Weidenfeller saw red early on, and his team almost managed to force the match into extra-time, though were finally sund in the 87th minute by a Vahid Hashemian strike.
Chaotic scenes then followed when the referee awarded Dortmund a penalty deep into injury time, though then went back on his decision after consulting with his linesman.
IN BRIEF
Elsewhere, goals from Roberto Hilbert, Matthieu Delpierre, Mario Gomez and Cacau gave Stuttgart a 4-2 victory over Babelsberg, while Pirmasens, who put out Werder Bremen in round one, saw their run come to an end with a 3-0 home defeat to Unterhaching.
There were goals aplenty on the North coast, where Burghausen beat Luebeck 5-4, while top flight sides VfL Bochum and Wolfsburg both eased to victories over lower league opposition.
