Championship side Derby County stunned Manchester United on penalties as Frank Lampard got one over on former boss Jose Mourinho in their first meeting in the dugout to progress in the Carabao Cup.
Marouane Fellaini appeared to have spoiled the party when he netted a 95th minute equaliser to secure a 2-2 draw for the 10-man hosts and a chance of redemption via spot-kicks. However, Phil Jones missed the 16th penalty to spark delirium amongst the visiting support.
With the pre-match build-up dominated by another row between Mourinho and Paul Pogba, who was omitted from the squad, United set about trying to make headlines on the pitch.
It took the hosts only three minutes to open the scoring, Juan Mata completing a fine sweeping move to give the Red Devils the best possible start. Romelu Lukaku spurned a couple of chances to extend United's lead before half-time, and Derby slowly grew into the contest.
After the break United had goalkeeper Sergio Romero to thank for maintaining their lead when Harry Wilson saw his shot saved from point-blank range, but the Argentina international could do nothing when the player on loan from Liverpool curled in a sensational free-kick from 30 yards only minutes later.
Things went from bad to worse for United when Romero was shown a straight red for handling Wilson's effort outside his box and Derby made their numerical advantage count, taking the lead through Jack Marriott's late header.
But Fellaini ensured the game would be decided in a shootout with his last-gasp header levelling the scores deep into injury-time, but it was all in vain as Derby ran out winners as Jones saw his tame penalty saved by Scott Carson.

TALKING POINT

It seems the drama at Old Trafford is never-ending. Three consecutive victories took the heat off Mourinho, but then his side mustered a dismal home draw with Wolves; Pogba made some silly post-match comments; Mourinho responded by telling the Frenchman he would never captain the side again; now they must recover after suffering an embarrassing home defeat to Championship opposition. It was hardly a competition on top of Mourinho's priority list, and defeat is unlikely to be the final nail in the coffin for the Portuguese, but it certainly doesn't help his cause.

Harry Wilson (Derby): Epitomised everything positive about Derby, the youngster producing a fearless, tireless and entirely inspirational performance at one of the biggest stadiums in world football. His stunning free-kick will go down as one of the best goals of the season.

3' - GOAL! Man United 1-0 Derby. A lovely sweeping move is finished by Mata! The attack stems from the flank where Martial is driving forward at defenders with pace and intent. His cross is dummied by Lukaku, Lingard works it to Mata who curls the ball into the bottom corner. Great start for United.
13' - MISS! Lukaku seizes on a defensive mistake from Keogh and suddenly the Belgian is through on goal with just the goalkeeper to beat. Somehow though he sticks it wide!
59' - GOAL! Man United 1-1 Derby. Wilson equalises with a sensational free-kick from all of 30 yards! It looks too far out to shoot, but the Derby youngster gets plenty of bend and dip in his effort and Romero is left rooted to the spot as it flies into the top corner. Unbelievable strike.
67' - ROMERO IS SENT OFF! The United goalkeeper races off his line and handles the ball outside his box to block Wilson's effort.
85' - GOAL! Man United 1-2 Derby. Oh my! Derby are ahead! After a quick check with VAR the visitors celebrate what could be a late, late winner. Marriott reacted quickest after Mount's shot was parried by Grant. The referee wanted to check if he was an offside position but it's going to count! Is there any way back for United?
90'+5 - GOAL! Man United 2-2 Derby. Fellaini with a trademark header levels the scores right at the death! A huge sigh of relief all around Old Trafford. Penalties are looming...
