Simone Perrotta gave the Giallorossi the lead just before the break after thoroughly outplaying Inter throughout the first half, though Marco Materazzi struck a controversial equaliser from the penalty spot on 52 minutes after an Adriano dive in the area.
It seemed as though Inter would preserve their unbeaten record, but a Totti free-kick in the 88th minute left the hosts stunned, and substitute Cassetti's injury time strike sealed a miserable day for the blue and black half of Milan.
Roma 'keeper Doni received what appeared to be a second booking for time-wasting in between the two late goals, though was not given his marching orders by the referee.
The defeat - Inter's first of the season in Serie A - will do little more than postpone their title celebrations, while Roma appear to have second place all but secured from city rivals Lazio.
Roma began the more adventurous of the two sides, and took the game to Inter, with both full backs and wingers looking to get forward at every opportunity.
They carved out the first chance of the match, as Mancini bought himself a yard of space against Douglas Maicon with a flurry of stepovers, though struck a rising shot across the face of goal when a pull back would have been more advisable.
Christian Wilhelmsson then hung up an inviting ball for Totti at the far post, though the captain was unable to keep his header down.
The wastefulness in possession of the Internazionale front pair of Adriano and Zlatan Ibrahimovic put their midfield under pressure, and a Dejan Stankovic strike from the edge of the area that cannoned up off the crossbar was the only threatening attempt on goal they could muster in the first half.
In contrast, Inter goalkeeper Julio Cesar had to be in inspired form to tip a vicious shot from Mancini onto the bar.
Roma finally grabbed the lead they deserved just before the break, with their prolific midfield conjuring yet another goal. Daniele de Rossi sent Christian Chivu flying down the left flank, and his cross went straight through the legs of Esteban Cambiasso, and was turned in from close range by Perrotta.
Inter were level soon after half-time though in the most controversial of circumstances, after Adriano dived in the area to win a penalty. The Brazilian striker nicked the ball away from Roma 'keeper Doni, who pulled out of the challenge, only for Adriano to hurl himself at the ground. Materazzi converted the penalty with aplomb.
The goal swung the momentum of the game back Inter's way, and it was they who took the initiative, and could have gone ahead on the hour mark were it not for a superb save low to his left from the otherwise disappointing Adriano at the near post, while Cambiasso was denied on the follow-up by desperate defending from the Roma back line.
As Roma'a challenge appeared to be diminishing, it seemed as though Inter would secure the title and preserve their unbeaten record in Serie A, but captain Totti had other ideas. He struck a free-kick off the end of the wall in the 88th minute that wrong footed Julio Cesar and left the Giuseppe Meazza in stunned silence.
Goalkeeper Doni was booked for time wasting moments later, seemingly for the second time in the game, though was allowed to remain on the pitch.
Casetti then wrapped up the win, picking out the bottom left hand corner with a low shot past the dive of Julio Cesar.
