A 35-year-old supporter was rushed to hospital for emergency surgery after being hit by a van but later died, while in a separate incident, four Napoli fans were stabbed in a clash around an hour before the game.
One of them was taken to hospital, and two Inter ultras have been arrested in connection to the attack.
The atmosphere during the match remained hostile, with Napoli manager Carlo Ancelotti saying afterwards that the game should have been suspended because of racist chanting directed at Kalidou Koulibaly.
Ancelotti said that they had asked three times for the game to be stopped due to the abuse, but that the only action taken was to make announcements to the crowd.
He said that the Senegalese player was put on edge by the crowd's behaviour and that his red card in the 81st minute "influenced" the outcome of the match.
"The next time we will stop playing and leave the pitch, even if we have to lose the match," Ancelotti told reporters.
