Trump had earlier come through a tricky semi-final against Shaun Murphy 4-2.
He started slowly in the final, but for the third time in five Players Tour Championship events this season he managed to recover from 3-1 down against Hamilton to eke out the win.
Hamilton, 39, has slipped to 42 in the world rankings, but beat up-and-coming Daniel Wells, Bjorn Haneveer and Marcus Campbell on his way to coming within a whisker of the title.
