"Palermo is a good tournament for me; I feel at home," said Medina, who also won the tournament in 2001.
"The match was not so easy, especially in the first set, I was down 4-2 but then I fought and I won the set."
"I'm very happy to have been in the final," said Koukalova, winner of one title from five finals.
"Anabel played very well and I was just a little tired. In the first set I was leading 4-2 but then she came back and plays very well.
"I'm happy anyway because it is my sixth final and I won my first tournament this year. Now I will go to the US, playing in San Diego, Los Angeles and preparing for the US Open."
Medina, ranked world number 30, is just three spots shy of her career-high.
"I'm happy to be No.1 in Spain but Conchita (Martinez) and Arantxa (Sanchez-Vicario) are still the best," said Medina.
"They were No.1 for 18 years, but they never won the Palermo tournament three times!"
