Pozzovivo (AG2R La Mondiale) attacked with two kilometres remaining and held on to finish three seconds clear of his six rivals. Rigoberto Uran (Etixx-QuickStep) and Dan Martin (Cannondale-Garmin) finished second and third respectively.
Pierre Rolland (Europcar) finished 20th, but still managed to swap places with overnight leader and fellow member of the stage one breakaway Maciej Paterski (Sprandi Polkowice) in the overall standings. Paterski finished 42nd, 1:34 adrift of the stage winner and 1:12 behind Rolland.
The Polish rider's slight slump was hardly surprising - the stage one victor completed the gruelling 293km Milan-San Remo in the lead pack on Sunday before heading straight to Spain to compete in the seven-stage race.
Britain's Chris Froome is 2:46 off the overall pace after he lost ground during the ascent of the Alt des Angels.
