Energie Cottbus became the second Bundesliga side to fall in the first round, losing 6-5 to third division Rot-Weiss Essen on penalties.
Bundesliga champions VfB Stuttgart survived a scare as they needed two penalties from Roberto Hilbert, the second a minute into stoppage time, to scrape a 2-1 win over SV Wehen Wiesbaden in a match that saw both sides reduced to nine men.
Six other Bundesliga clubs won their first round matches against teams in lower divisions on the first weekend of competition in the new season after a three-month break.
In the biggest upset of the first round, Fabian Boll scored in the 87th minute for St. Pauli, regular giantkillers who reached the semi-finals two years ago.
For Leverkusen, who finished fifth last season in the Bundesliga and qualified for the UEFA Cup, it was their first Cup exit in the first round in 11 years in a match played in front of a crowd of 15,000 in Hamburg.
"It's fantastic to beat a team of millionaires," said Boll.
Werder Bremen avoided embarrassment when a Boubacar Sanogo goal in the 90th minute gave them a 1-0 win over third division side Eintracht Braunschweig.
Borussia Dortmund beat FC Magdeburg 4-1, Hanover 96 defeated Rot-Weiss Ahlen 3-1, and Hertha Berlin came safely through 3-0 against SpVgg Unterhaching. Hansa Rostock crushed fourth division side Rot-Weiss Hasborn 8-0 while Kaiserslautern beat SV Wilhelmshaven, another fourth division side, 4-0.
