Hazard’s team-mate Diego Costa, Manchester United goalkeeper David De Gea, Arsenal forward Alexis Sanchez and Liverpool’s Philippe Coutinho are the other men included.
Four of those players - Hazard, Kane, De Gea and Coutinho - also make the PFA Young Player of the Year shortlist. They are joined by Chelsea stopper Thibaut Courtois and Liverpool's Raheem Sterling.
Hazard won the Young Player accolade last season, but missed out on the main prize to former Liverpool striker Luis Suarez. The Belgian has flourished under Jose Mourinho this campaign, playing a pivotal role as Chelsea have opened up a seven-point gap at the Premier League summit with a game in hand and seven matches to go.
Kane only broke into the Spurs starting XI in November, but has not looked back as a haul of 19 league goals helped earn him a call up into Roy Hodgson's England squad.
Manchester City are without representation in either category, despite possessing one of the Premier League's joint-top goalscorers in Sergio Aguero. The Argentine is tied with Costa and Kane at the top of the charts.
Men's PFA Players' Player of the Year: Diego Costa (Chelsea), David de Gea (Manchester United), Philippe Coutinho (Liverpool), Eden Hazard (Chelsea), Harry Kane (Tottenham Hotspur), Alexis Sanchez (Arsenal).
Men's PFA Young Player of the Year: Thibaut Courtois (Chelsea), Philippe Coutinho (Liverpool), David de Gea (Manchester United), Eden Hazard (Chelsea), Harry Kane (Tottenham Hotspur), Raheem Sterling (Liverpool).
