The 21-year-old Wolves winger is back at Burton for a second loan spell but has only featured as a substitute on three occasions for Jimmy Floyd Hasselbaink's League One side.
The Brewers are third in the table but have only been able to use Ismail sparingly and he has now suffered another blow.
Having spent part of the 2013/14 season at Burton, the England youth international was injured in a reserve match against Port Vale - with Hasselbaink insisting he still wants to keep Ismail at the club past the end of his current loan deal, which runs until January.
"If he comes back (to fitness) we would want to extend his loan," the Dutchman told Burton's official website.
"He was only going to play 60 minutes but got injured in the 55th minute. He has a bad ankle after a tackle and that is a blow.
"He was doing well and it's a blow for the lad as he has had some injuries but was really on the right track. We had him in that really good spot where he was getting his minutes and making a difference. He was putting in nice crosses and giving us a different edge.
"Now he will have to start from zero again, unfortunately. But that's what it is in football."
