Newcastle manager Alan Pardew's decision to introduce Cisse for Remy Cabella at half-time proved inspired as two opportunist finishes on 57 and 78 minutes condemned Jose Mourinho's side to their first defeat of the campaign.
Newcastle had to see out 10 minutes of normal and six minutes of stoppage time with 10 men after Steven Tayor collected a second yellow card for a late challenge on visiting substitute Andre Schurrle.
Chelsea sub Didier Drogba had headed home on 83 minutes to give his side hope of rescuing a point, but Newcastle held on for a famous win that denied Chelsea the chance to move nine points clear at the summit.
Newcastle also had to complete the job with their third goalkeeper Jak Alnwick, who made his full debut at half-time after an injury to Rob Elliot.
But like so many men in black and white, Alnwick was in inspired form for his team as Newcastle held on to end Chelsea's record-equalling 23-game unbeaten run.
Manchester City can close the gap to only three points at the top if they complete a home win over Everton later in the afternoon.

KEY MOMENTS
13' - CHANCE! Courtois off his line quickly to collect a cross before Chelsea break to the other end as Hazard sends a shot wide of goal. Really decent cross from Janmaat there.
20' - CHANCE! Willian sends a shot spinning inches wide of the home goal. Lovely play by Hazard. Failed to pick out Costa, but Willian's effort so unfortunate not to find the net.
23' - CHANCE! Fabregas with a deflected effort wide of goal. Chelsea work the corner shot, but Ameobi doing well to block the Oscar volley.
26' - CHANCE! Costa drills an effort at goal, but straight into the arms of Elliot in the Newcastle goal. Wasn't the best of finishes by the Spanish striker.
32' - CHANCE! Excellent save by Courtois to deny Colback after Perez had played in him. Brilliant stuff from Newcastle, but the Chelsea goalkeeper showing his undoubted class.
57' - GOAL! Cisse scores for Newcastle. Cahill fails to block the Ameobi cross, and that is an expert finish from Cisse. A 1-0 lead for Newcastle.
78' - GOAL! Fine finish from Cisse after Chelsea failed to deal with the Sissoko effort. Chelsea cut open on the break. 2-0. Surely game over now.
81' RED CARD! Steven Taylor takes out Schurrle with a late sliding challenge. Second booking for him, and that means red. The home team lead 2-0, but are down to 10 men.
83' GOAL! Drogba rises above Alnwick to head into the net from the Fabregas free-kick. What a finish we have here. 2-1 now to the 10 men of Newcastle.

MAN OF THE MATCH
Fabricio Coloccini (Newcastle United): Seemed to be everywhere for the home side as he helped Newcastle complete a rousing victory as Chelsea threw the kitchen sink at the home team late on.
TALKING POINT
Do we have a title race? Chelsea remain strong favourites for the title, but this was a timely reminder that they cannot take anything for granted in the Premier League. Probably deserved a draw from their frantic finish to the match, but Newcastle's tactics were spot on.
