De Bruyne has suffered a number of injuries this season, including to both of his knees and his hamstring.
It has seen City's talisman miss nearly a combined 20 weeks of action in total across this season.
Despite taking an early lead through academy product Phil Foden, a nervous hush fell over the Etihad when De Bruyne went down after taking a shot just before the break.
The Belgian clutched his left side and his left knee was checked by the medical staff.
He was able to walk off but City fans face an anxious wait to see the extent of the injury.
