The top seed lost 11 out of 12 games from near the end of the first set to the start of the third before capturing her fifth WTA Tour title.
Medina Garrigues raced to a 5-1 lead in the opening set but lost the next three games before holding her serve to take the set.
Cho won every game in the second set to square the match and then took the first two games in the decider.
Medina Garrigues was showing signs of tiredness after her marathon win over Israel's Shahar Peer in Thursday's semi-finals -- the sixth longest recorded match in women's tennis history-- but rebounded to win the last six games.
"I am very happy but I am very tired also," she told reporters.
"You always have to think you can win but you also have to know that it will be very tough."
