Sergio Ramos, Toni Kroos, Luka Modric and Cristiano Ronaldo all feature in the side, which also includes Barcelona trio Gerard Pique, Andres Iniesta and Lionel Messi.
However, there is no place for Real and Wales forward Gareth Bale, while Barcelona pair Luis Suarez and Neymar also miss out.
Juventus duo Gigi Buffon and Leonardo Bonucci are the Serie A representatives in the line-up.
Bayern Munich's Jerome Boateng completes the back four, while Atletico Madrid's Antoine Griezmann joins Ronaldo and Messi up front in the combined XI.
No Premier League player makes the shortlist once again - with Bale the last to do so outright in 2011 while he was at Tottenham.
Angel Di Maria and Mesut Ozil have featured in the XI since, but it was their achievements for Real Madrid which saw them earn a place in the team.
UEFA Team of the Year: Buffon; Ramos, Bonucci, Pique, Boateng; Modric, Kroos, Iniesta; Messi, Ronaldo, Griezmann
