Cilic, contesting his third Queen’s final, took the opening set 6-4 but Lopez fought his way back into the contest to take the next two sets on a tie-break, winning 4-6 7-6(2) 7-6(8)
“It’s tough to believe that at 35 years of age I am playing my best tennis, but I am,” said Lopez. "There's no better preparation for Wimbledon... I hope to see you here next year guys."
The third-set finale was a dramatic affair.
Lopez, who suffered a heartbreaking defeat in the 2014 final when he held a match point against Grigor Dimitrov, this time saved a match point in a tense final set tiebreak before toppling the Croat.
Lopez saved his match point with a steely volley at 5-6 in the breaker before Cilic fended off two. Then, at the third time of asking, crowd favourite Lopez could celebrate after fourth seed Cilic pulled a forehand into the tramlines.
The Spaniard is tuning up for Wimbledon in impressive fashion, having made the final in Stuttgart last week, only to lose to Lucas Pouille.
Lopez will harbour real hopes of beating his best effort at Wimbledon - reaching the quarter-finals on three occasions.
A gracious Cilic said: “Big congratulations to him he is always playing well on grass and he deserves a title."
"I played great tennis this week and enjoyed every single day… it was an amazing week and great preparation for Wimbledon."
